Understanding Lymphoedema
13 Mar 2026Persistent Limb Swelling — Causes, Signs, and Management
What Is Lymphoedema?
Lymphoedema is a chronic condition characterised by persistent swelling of a limb due to impaired lymphatic drainage. The lymphatic system normally removes excess fluid, proteins, and waste products from body tissues. When this system is damaged or obstructed, fluid accumulates — most commonly in the legs — resulting in progressive swelling that does not fully resolve with rest.
What Causes Lymphoedema?
Lymphoedema may be classified as:
- Primary lymphoedema — arising from congenital or developmental abnormalities of the lymphatic vessels, often presenting in early adulthood
- Secondary lymphoedema — developing as a consequence of surgery, radiation therapy, infections (including filariasis), trauma, or cancer treatment
Secondary lymphoedema is the more common form and can arise at any stage of life following a triggering event.
Early Signs to Watch For
Unlike ordinary swelling, lymphoedema does not completely resolve with rest or leg elevation. Key signs include:
- Persistent swelling of one or both legs
- A sensation of heaviness, tightness, or fullness in the limb
- Gradual skin thickening over affected areas
- Recurrent skin infections (cellulitis)
Recognising these signs early and seeking evaluation promptly can make a significant difference to long-term outcomes.
Why Early Diagnosis Matters
If left untreated, lymphoedema can gradually worsen, leading to progressive skin changes, recurrent infections, and reduced mobility. Early intervention — before significant tissue changes develop — can substantially slow disease progression and preserve quality of life.
Management and Treatment
While lymphoedema is a chronic condition, it can be effectively managed with a structured care plan. Treatment options include:
- Compression therapy using specialised stockings or multi-layer bandaging
- Manual lymphatic drainage — a specialised massage technique to encourage fluid movement
- Tailored exercise programmes to support lymphatic flow
- Meticulous skin care to prevent infections and skin breakdown
- Surgical interventions in carefully selected cases
With structured care and regular follow-up, most patients with lymphoedema can lead active, productive lives.
